Overview
- Provide compassionate home care services, focusing on personalized care planning and community health education.
- Conduct thorough initial assessments, including physical evaluations and medical history reviews, to determine client needs.
- Develop and implement individualized care plans based on nursing diagnoses and therapeutic goals.
- Deliver direct client care, administer medications, and perform rehabilitative nursing procedures with precision.
- Collaborate with physicians to update care plans and address changes in client conditions effectively.
- Provide health education and counseling to clients and families to achieve care plan objectives.
- Document clinical notes and maintain accurate records in compliance with agency policies.
- Participate in staff meetings to share insights and stay informed about updates and best practices.
Key Responsibilities & Duties
- Conduct initial client assessments, including physical evaluations and medical history documentation.
- Develop and revise care plans to address evolving client needs and health status changes.
- Administer prescribed medications and treatments, ensuring adherence to physician orders.
- Provide direct care and preventive nursing procedures to clients in their homes.
- Coordinate client care as a Case Manager, managing assigned caseloads effectively.
- Prepare and update clinical notes, communicating with physicians as needed.
- Educate clients and families on health care practices and care plan objectives.
- Participate in staff meetings and contribute to team discussions and skill-sharing.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Current RN licensure in the state and CPR certification.
- Minimum of 1-2 years of recent acute care or home care experience preferred.
- Strong communication, observation, and problem-solving skills.
- Ability to supervise home health aides and manage client care effectively.
- Proficient in basic math and computer knowledge helpful.
- Physical capability for prolonged walking, standing, and lifting clients and equipment.
- Valid driver’s license and ability to drive in various weather conditions.
