Home Care Nurse

Overview

• Provide compassionate home care services, focusing on personalized care planning and community health education.
• Conduct thorough initial assessments, including physical evaluations and medical history reviews, to determine client needs.
• Develop and implement individualized care plans based on nursing diagnoses and therapeutic goals.
• Deliver direct client care, administer medications, and perform rehabilitative nursing procedures with precision.
• Collaborate with physicians to update care plans and address changes in client conditions effectively.
• Provide health education and counseling to clients and families to achieve care plan objectives.
• Document clinical notes and maintain accurate records in compliance with agency policies.
• Participate in staff meetings to share insights and stay informed about updates and best practices.

Key Responsibilities & Duties

• Conduct initial client assessments, including physical evaluations and medical history documentation.
• Develop and revise care plans to address evolving client needs and health status changes.
• Administer prescribed medications and treatments, ensuring adherence to physician orders.
• Provide direct care and preventive nursing procedures to clients in their homes.
• Coordinate client care as a Case Manager, managing assigned caseloads effectively.
• Prepare and update clinical notes, communicating with physicians as needed.
• Educate clients and families on health care practices and care plan objectives.
• Participate in staff meetings and contribute to team discussions and skill-sharing.

Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited institution.
• Current RN licensure in the state and CPR certification.
• Minimum of 1-2 years of recent acute care or home care experience preferred.
• Strong communication, observation, and problem-solving skills.
• Ability to supervise home health aides and manage client care effectively.
• Proficient in basic math and computer knowledge helpful.
• Physical capability for prolonged walking, standing, and lifting clients and equipment.
• Valid driver’s license and ability to drive in various weather conditions.
