Key Responsibilities & Duties

• Conduct comprehensive patient screenings, examinations, diagnoses, and treatments across various age groups and health conditions.
• Provide evidence-based care tailored to individual patient needs, ensuring compassion and respect.
• Collaborate with healthcare professionals to deliver seamless and coordinated patient care.
• Refer patients to internal and external specialists to address specific health needs effectively.
• Participate in case conferences and contribute to organizational quality improvement initiatives.
• Engage in continuing medical education to stay updated on healthcare advancements and best practices.
• Document patient interactions and care promptly and accurately using EPIC systems.
• Ensure efficiency and patient satisfaction by adhering to scheduled appointments and providing timely follow-ups.

Job Requirements

• Doctor of Medicine (MD) with active New York State Certification/Licensure.
• Current NYS Registration and DEA Controlled Substances Registration Certificate.
• Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
• Minimum of 2 years of primary care experience; 3 years preferred.
• Experience in community-based primary care settings is highly desirable.
• Proficiency in bilingual communication (Spanish/English) is a plus.
• Strong collaboration skills within a team-based care model.
