Key Responsibilities & Duties
- Conduct comprehensive patient screenings, examinations, diagnoses, and treatments across various age groups and health conditions.
- Provide evidence-based care tailored to individual patient needs, ensuring compassion and respect.
- Collaborate with healthcare professionals to deliver seamless and coordinated patient care.
- Refer patients to internal and external specialists to address specific health needs effectively.

Job Requirements
- Doctor of Medicine (MD) with active New York State Certification/Licensure.
- Current NYS Registration and DEA Controlled Substances Registration Certificate.
- Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 2 years of primary care experience; 3 years preferred.
- Experience in community-based primary care settings is highly desirable.
- Proficiency in bilingual communication (Spanish/English) is a plus.
- Strong collaboration skills within a team-based care model.
