Job Description

Overview

  • Provide nursing care and assessments for adult and elderly populations in a community-based Adult Day Health Care Program.
  • Collaborate with a multidisciplinary team to deliver comprehensive health services and education.
  • Conduct nursing assessments, develop treatment plans, and monitor registrants' health progress.
  • Coordinate healthcare services and act as a liaison with private physicians and medical directors.
  • Ensure safety, infection control, and proper handling of medical emergencies within the facility.
  • Provide individual and group health education and counseling to registrants and their families.
  • Maintain medical equipment and supplies, ensuring optimal operational conditions.
  • Participate in quality assurance and performance improvement initiatives.
  • Support registrants' rights and confidentiality, adhering to security policies and protocols.

Key Responsibilities & Duties

  • Perform nursing assessments and develop individualized treatment plans for registrants.
  • Monitor registrants' medications and ensure adherence to inventory standards.
  • Handle medical emergencies and educate staff on proper procedures.
  • Maintain a safe and clean environment, ensuring medical equipment is in optimal condition.
  • Provide health counseling and lead educational sessions for registrants and families.
  • Supervise nursing students and volunteers, fostering professional development.
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
  • Participate in staff meetings, quality assurance activities, and program improvement initiatives.
  • Assist with general tasks, including escorting registrants and serving meals.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Current NYS Registered Nurse license with 3-5 years of relevant experience.
  • Proficiency in MS Word and familiarity with UAS-NY certification preferred.
  • Experience in Med/Surg, Long Term Care, or Community Health settings.
  • Strong communication, documentation, and assessment skills.
  • Ability to handle physical demands, including lifting, transferring, and pushing registrants.
  • Commitment to maintaining confidentiality and adhering to security policies.
  • Warm personality with a passion for working with adult and elderly populations.
  • Availability for on-site work in a community-based healthcare setting.
