Overview
- Provide nursing care and assessments for adult and elderly populations in a community-based Adult Day Health Care Program.
- Collaborate with a multidisciplinary team to deliver comprehensive health services and education.
- Conduct nursing assessments, develop treatment plans, and monitor registrants' health progress.
- Coordinate healthcare services and act as a liaison with private physicians and medical directors.
- Ensure safety, infection control, and proper handling of medical emergencies within the facility.
- Provide individual and group health education and counseling to registrants and their families.
- Maintain medical equipment and supplies, ensuring optimal operational conditions.
- Participate in quality assurance and performance improvement initiatives.
- Support registrants' rights and confidentiality, adhering to security policies and protocols.
Key Responsibilities & Duties
- Perform nursing assessments and develop individualized treatment plans for registrants.
- Monitor registrants' medications and ensure adherence to inventory standards.
- Handle medical emergencies and educate staff on proper procedures.
- Maintain a safe and clean environment, ensuring medical equipment is in optimal condition.
- Provide health counseling and lead educational sessions for registrants and families.
- Supervise nursing students and volunteers, fostering professional development.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
- Participate in staff meetings, quality assurance activities, and program improvement initiatives.
- Assist with general tasks, including escorting registrants and serving meals.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Current NYS Registered Nurse license with 3-5 years of relevant experience.
- Proficiency in MS Word and familiarity with UAS-NY certification preferred.
- Experience in Med/Surg, Long Term Care, or Community Health settings.
- Strong communication, documentation, and assessment skills.
- Ability to handle physical demands, including lifting, transferring, and pushing registrants.
- Commitment to maintaining confidentiality and adhering to security policies.
- Warm personality with a passion for working with adult and elderly populations.
- Availability for on-site work in a community-based healthcare setting.
