Overview
- Provide skilled nursing care to patients in their homes, ensuring comprehensive and compassionate healthcare delivery.

- Administer treatments and procedures as directed by physician orders, maintaining high standards of care.
- Monitor patient progress and adjust care plans accordingly to optimize outcomes and satisfaction.
- Ensure accurate documentation and compliance with regulatory standards and organizational protocols.
- Supervise home health aides, providing guidance and conducting required supervisory visits.
- Educate patients and families on medication management and health practices to promote wellness.
- Maintain effective communication with physicians and care teams to address changes in patient conditions.
Key Responsibilities & Duties
- Conduct comprehensive patient assessments to determine healthcare needs and develop care plans.
- Administer medications, treatments, and procedures in accordance with physician directives.
- Perform wound care, ostomy care, Foley catheter management, and G-tube care.
- Monitor and document patient progress, ensuring compliance with care plans and regulatory requirements.
- Coordinate care with interdisciplinary teams to ensure seamless and effective healthcare delivery.
- Supervise and support home health aides, conducting supervisory visits as required.
- Educate patients and families on healthcare practices, medication management, and wellness strategies.
- Respond promptly to changes in patient conditions, communicating with physicians and care teams.
Job Requirements
- Active Registered Nurse (RN) license in New Jersey.
- Bachelor of Science in Nursing (BSN) preferred; Associate’s Degree in Nursing required.
- AHA Basic Life Support (BLS) certification for healthcare providers.
- Minimum of 6 months RN experience or 1 year LPN experience.
- Proficiency in clinical skills such as wound care, ostomy care, and G-tube management.
- Strong preference for candidates with home care, hospice, or med-surg experience.
- Ability to lift, push, or pull up to 50 lbs and stand for extended periods.
- Excellent clinical judgment, documentation skills, and time management abilities.
