Key Responsibilities & Duties

• Perform detailed evaluations to assess physical conditions, pain levels, and functional limitations.
• Develop and implement individualized care plans with specific therapeutic goals.
• Provide hands-on therapy sessions and monitor patient progress effectively.
• Collaborate with physicians and healthcare teams to align treatment plans with overall patient care objectives.
• Ensure precise and timely documentation of therapy sessions and patient outcomes.
• Utilize telehealth platforms to deliver remote rehabilitation services when applicable.
• Participate in case management and resource coordination to optimize patient care efficiency.
• Adhere to all regulatory requirements and maintain compliance with healthcare standards.

Job Requirements

• Associate of Applied Science (AAS) in Physical Therapy or related field.
• Active New York State License and registration as a Physical Therapist.
• Proficiency in conducting OASIS evaluations and maintaining accurate documentation.
• Experience in telehealth services for rehabilitation is preferred.
• Strong interpersonal skills to collaborate effectively with patients and teams.
• Commitment to delivering patient-centered care and achieving recovery goals.
• Flexibility to adapt to varying patient needs and schedules.
• Previous experience in physical therapy is advantageous but not mandatory.
