Job Description

Overview

  • Deliver compassionate and effective nursing care to clients in a supportive healthcare setting, ensuring their physical and emotional well-being.
  • Coordinate medical case management, including treatment plans and referrals, to optimize client health outcomes.
  • Conduct health assessments, manage medication administration, and ensure compliance with healthcare protocols and standards.
  • Collaborate with multidisciplinary teams to provide comprehensive and client-centered care solutions.
  • Educate clients on health practices and provide emergency medical assistance when required.
  • Engage in professional development opportunities to enhance skills and knowledge in nursing practices.
  • Work onsite during designated hours, including weekends, to ensure consistent healthcare coverage for clients.

Key Responsibilities & Duties

  • Supervise healthcare staff and coordinate client care within a collaborative and multidisciplinary team environment.
  • Facilitate access to essential medical and psychosocial resources, ensuring effective referrals and treatment coordination.
  • Perform regular health evaluations and collaborate with nursing leadership to address client health needs.
  • Maintain accurate and detailed documentation, including medication records and controlled substance logs.
  • Advocate for clients' health preferences and goals, fostering a supportive care environment.
  • Participate in treatment team meetings and case conferences to develop and implement optimal care strategies.
  • Provide emergency medical assistance and deliver tailored health education to clients.
  • Perform additional duties as assigned to support the healthcare team and enhance client outcomes.

Job Requirements

  • Licensed Practical Nurse (LPN) certification is mandatory for this position.
  • Associate of Applied Science (AAS) degree in nursing or a related field is required.
  • Minimum of 1 year of experience in a healthcare setting; 3 years preferred.
  • Proficiency in medical case management and client health advocacy practices.
  • Experience in maintaining medication records and controlled substance logs accurately.
  • Ability to provide emergency first aid and CPR assistance effectively.
  • Strong communication and teamwork skills to collaborate with healthcare professionals.
