Overview

  • Provide compassionate mental health care in a collaborative, patient-centered environment, ensuring effective treatment plans and holistic support.

Key Responsibilities & Duties

  • Conduct telephonic outreach and case management to support patient engagement and care continuity.
  • Perform clinical assessments and interventions tailored to individual mental health needs.
  • Develop and implement evidence-based treatment protocols for chronic behavioral conditions.
  • Provide referrals to community resources and support services to enhance patient outcomes.

Job Requirements

  • Master’s Degree in Social Work or related field is required.
  • Licensure as LMSW, LCSW, PhD, or PsyD is preferred.
  • Experience working with diverse populations, including individuals with complex needs.
  • Proficiency in electronic health records and Microsoft Office applications.
  • Bilingual Spanish skills are preferred, with additional compensation available.
  • Strong knowledge of behavioral medicine and evidence-based treatments for mental health conditions.
  • Ability to design and implement clinical pathways for effective care management.
  • Excellent organizational, interpersonal, and communication skills to foster collaboration.
