Job Description

Overview

  • Lead and oversee the coordination of in-home care services, ensuring compliance with healthcare standards and delivering exceptional patient care.
  • Manage a team of coordinators, providing mentorship, training, and fostering professional growth opportunities within the department.
  • Address escalations involving patients, families, and providers to maintain service excellence and satisfaction.
  • Implement process improvements and quality initiatives to enhance service delivery and operational efficiency.
  • Collaborate with internal and external stakeholders to ensure seamless care transitions and patient safety.
  • Ensure compliance with caregiver certifications, training, and regulatory requirements to uphold service standards.
  • Monitor team productivity and adherence to workflows, introducing enhancements as necessary.
  • Prepare and present departmental updates, metrics, and reports to leadership for strategic planning.

Key Responsibilities & Duties

  • Supervise daily operations of the coordination department, ensuring timely and accurate caregiver scheduling.
  • Provide constructive feedback, training, and mentorship to coordinators to support their professional development.
  • Oversee case assignments, urgent coverage needs, and crisis management to minimize service disruptions.
  • Collaborate with Intake, Compliance, HR, and Field Operations teams for smooth onboarding and care transitions.
  • Audit schedule entries and caregiver records to ensure data accuracy and compliance with standards.
  • Resolve scheduling conflicts, caregiver concerns, and client service issues promptly and effectively.
  • Monitor team adherence to processes and performance indicators, identifying areas for improvement.
  • Promote a collaborative and accountable work environment, encouraging communication and teamwork.

Job Requirements

  • Bachelor of Science (BS) degree or equivalent educational background in a relevant field.
  • Minimum of 3 years of experience in home care coordination or healthcare scheduling roles.
  • Preferred 5 years of experience, including supervisory roles within healthcare settings.
  • Proficiency in scheduling software such as HHAeXchange or similar platforms.
  • Strong understanding of state healthcare regulations and Medicaid/MLTC processes.
  • Excellent organizational, time management, and leadership skills.
  • Effective problem-solving and conflict resolution capabilities.
  • Bilingual proficiency in English and Spanish, Russian, or Cantonese is advantageous.
