Job Description

Overview

  • Provide high-quality outpatient physical therapy services to patients with orthopedic, neurologic, and musculoskeletal conditions.
  • Deliver individualized treatment plans focused on restoring function, improving strength, and enhancing independence.
  • Educate patients, families, and caregivers throughout the therapy process to ensure understanding and compliance.
  • Collaborate with interdisciplinary teams to support patient goals and optimize outcomes.
  • Participate in program development, clinical education, and process improvement initiatives.
  • Work in a supportive and team-oriented environment that values professional growth and quality care.
  • Utilize electronic health records to document evaluations, treatments, and progress effectively.
  • Engage in department initiatives, committees, and quality improvement efforts to enhance services.

Key Responsibilities & Duties

  • Perform evaluations and develop, implement, and adjust treatment plans for patients.
  • Establish functional baselines and track patient progress throughout therapy sessions.
  • Supervise Physical Therapist Assistants, students, and rehab support staff as applicable.
  • Provide treatment under the direction of a licensed Physical Therapist, ensuring adherence to established plans.
  • Support patient participation in individual and group therapy activities to enhance recovery.
  • Document evaluations, treatments, and progress in compliance with timelines and standards.
  • Communicate barriers to timely documentation and collaborate to resolve issues.
  • Participate in clinical education and serve as a resource within areas of expertise.

Job Requirements

  • Bachelor of Science degree from an accredited Physical Therapy program.
  • Active Physical Therapist or Physical Therapist Assistant license in the State of Michigan.
  • BLS certification required within three months of hire.
  • Minimum of one year of experience, with three years preferred.
  • Strong communication and interpersonal skills to foster patient relationships.
  • Proficiency in electronic health records and basic computer applications.
