Overview

Key Responsibilities & Duties
- Support the Associate Director in managing daily operations of hospice nursing services and team coordination.
- Mentor RN Case Managers in developing effective care plans and symptom management strategies.
- Schedule and coordinate staff assignments to ensure optimal service delivery and patient satisfaction.
- Evaluate nursing documentation for compliance with organizational and regulatory standards.
- Conduct community presentations to promote hospice services and support outreach efforts.
- Provide leadership and guidance to nursing staff, fostering professional development and excellence.
- Respond to incidents beyond core hours to ensure continuity of care and patient satisfaction.

Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution required.
- Minimum of five years of hospice nursing experience; leadership experience preferred.
- Active New York State RN licensure required; CHPN certification preferred or obtainable within one year.
- Proficiency in Word, Outlook, and Windows applications essential for documentation and communication.
- Strong leadership and mentoring skills to guide and develop nursing staff effectively.
- Experience in triage, after-hours, or on-call home care settings highly valued.
- Availability to address incidents and ensure care continuity beyond standard hours.
