Overview

Key Responsibilities & Duties
- Assess, plan, implement, and evaluate patient care plans for effective delivery.
- Coordinate care across disciplines including physicians, nursing, and ancillary services.
- Facilitate admissions, transfers, and discharges ensuring appropriate care levels.
- Collaborate with leadership to address care progression and discharge barriers.
- Educate patients and families on treatment options and post-care services.
- Maintain compliance with state, federal, and accreditation standards.
- Participate in quality improvement initiatives and interdisciplinary care teams.
- Communicate effectively with physicians, nursing leadership, and external providers.
- Support a culture of patient-centered care aligned with organizational values.
Job Requirements
- Active California Registered Nurse (RN) license in good standing.
- Minimum 2 years of acute care nursing experience.
- Bachelor of Science in Nursing (BSN) required.
- Knowledge of case management principles and discharge planning.
- Strong clinical judgment, critical thinking, and organizational skills.
- Preferred certifications such as CCM, ACM, or CMAC.
- Experience with Medicare, Medi-Cal, and managed care populations preferred.
- Familiarity with InterQual or MCG guidelines is advantageous.
- Effective communication and interpersonal skills for patient advocacy.
