Job Description

Overview

  • Provide pharmaceutical care in an outpatient oncology setting, ensuring safe and effective medication management for patients undergoing cancer treatment.
  • Collaborate with healthcare professionals to optimize patient outcomes through evidence-based pharmacotherapy practices.
  • Monitor and manage complex drug regimens, including chemotherapy and supportive care medications.
  • Educate patients and caregivers on medication usage, side effects, and adherence strategies.
  • Participate in interdisciplinary team meetings to contribute to patient care plans.
  • Ensure compliance with regulatory standards and institutional policies for oncology pharmacy services.
  • Engage in continuous professional development and quality improvement initiatives within the pharmacy department.
  • Support the implementation of clinical protocols and guidelines for oncology pharmacotherapy.

Key Responsibilities & Duties

  • Dispense and compound medications, including chemotherapy and parenteral nutrition products, following established protocols.
  • Supervise pharmacy technicians and ensure accurate medication preparation and distribution.
  • Conduct pharmacokinetic consultations and renal dosing adjustments as per hospital protocols.
  • Monitor drug therapies for contraindications, interactions, and appropriateness of use.
  • Provide clinical consultations and drug information to healthcare providers and patients.
  • Participate in quality improvement activities and medication use reviews.
  • Contribute to antibiotic and opioid stewardship programs and manage medication shortages effectively.
  • Precept pharmacy residents and students, fostering their professional growth.

Job Requirements

  • Doctor of Pharmacy (PharmD) degree required; Board certification or residency completion preferred.
  • Minimum of 2 years of experience in hospital or oncology pharmacy; 3 years preferred.
  • Active pharmacy state licensure required within 60 days of hire.
  • BLS or ACLS certification required; PALS certification for pediatric practice preferred.
  • Proficiency in compounding sterile and non-sterile products, including chemotherapy.
  • Strong knowledge of pharmacokinetics, drug interactions, and oncology pharmacotherapy.
  • Excellent communication skills for patient education and interdisciplinary collaboration.
