Registered Nurse Clinical Hospice Liaison

Overview

• Serve as a Clinical Hospice Liaison, bridging healthcare facilities and hospice services to ensure quality patient care.
• Utilize clinical expertise to educate referral sources, families, and patients about hospice services and benefits.
• Engage in outreach activities to identify and establish new referral sources within the healthcare community.
• Collaborate with healthcare professionals to promote hospice care and maintain strong relationships.

Key Responsibilities & Duties

• Identify key decision-makers in healthcare facilities and build positive working relationships.
• Promote hospice services through regular visits and presentations to healthcare providers.
• Develop and implement strategies to increase referrals and census growth.
• Conduct initial meetings with patients and families to explain hospice services and obtain consent.
• Collaborate with clinical staff to ensure seamless patient transitions to hospice care.
• Provide education and resources to referral sources about hospice philosophy and services.
• Maintain accurate records of outreach activities and referral outcomes.
• Participate in team meetings and contribute to strategic planning for hospice growth.

Job Requirements

• Bachelor’s degree in Nursing (BSN) or related healthcare field preferred.
• Current, unencumbered Registered Nurse license required.
• Minimum of one year of sales or marketing experience in healthcare.
• Experience in hospice or palliative care marketing strongly preferred.
• Proven ability to establish and maintain relationships with healthcare professionals and facilities.
• Strong communication and empathetic skills to support patients and families.
• Reliable transportation, valid driver’s license, and auto insurance required.
• Ability to work independently and manage multiple priorities effectively.
