Job Description

Overview

  • Provide compassionate hospice care to patients in home and assisted living settings, ensuring comfort and dignity during end-of-life care.
  • Manage a caseload of 15–20 patients weekly, conducting 4–5 visits daily to deliver personalized care.
  • Perform clinical tasks such as symptom management, wound care, and Foley catheter insertion with precision and empathy.
  • Educate patients and caregivers on disease progression, symptom control, and end-of-life care strategies.
  • Coordinate care across interdisciplinary teams, ensuring seamless service delivery and patient advocacy.
  • Document patient interactions and care plans accurately, adhering to regulatory and organizational standards.
  • Supervise Home Health Aides, providing guidance and conducting required supervisory visits.
  • Participate in case conferences and contribute to interdisciplinary care planning for optimal patient outcomes.

Key Responsibilities & Duties

  • Deliver skilled nursing care to hospice patients in their homes, ensuring comfort and dignity.
  • Conduct comprehensive assessments and develop individualized care plans tailored to patient needs.
  • Perform medication reconciliation and coordinate care with physicians and healthcare teams.
  • Monitor patient progress, adjusting care plans to address evolving health conditions.
  • Educate patients and families on disease management, medications, and symptom control techniques.
  • Ensure timely and accurate documentation of patient care activities and outcomes.
  • Coordinate services and act as a liaison between patients, families, and healthcare providers.
  • Supervise Home Health Aides and conduct required supervisory visits to ensure quality care.
  • Participate in interdisciplinary case conferences to optimize care planning and delivery.

Job Requirements

  • Active New Jersey Registered Nurse (RN) License and AHA BLS Certification required.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing or Nursing Diploma acceptable.
  • Minimum 1 year of RN experience or 6 months of LPN experience required.
  • Proficiency in computer systems and documentation essential for accurate record-keeping.
  • Experience in home care, hospice, or community health nursing preferred.
  • Strong interpersonal skills for patient and caregiver education and advocacy.
  • Ability to coordinate care across interdisciplinary teams and manage complex cases effectively.
