Overview
- Provide compassionate hospice care to patients in home and assisted living settings during evening shifts.
- Manage a caseload of 15–20 patients weekly, ensuring personalized care and support.
- Focus on symptom management, patient education, and end-of-life care advocacy.
- Collaborate with interdisciplinary teams to coordinate comprehensive care plans.
- Perform clinical tasks such as wound care, Foley catheter management, and vital sign monitoring.
- Ensure compliance with regulatory standards and accurate documentation practices.
- Participate in case conferences and contribute to care planning discussions.
- Support patients and caregivers with education on disease progression and symptom control.
Key Responsibilities & Duties
- Deliver skilled nursing care to hospice patients in their homes and community settings.
- Conduct comprehensive assessments and develop individualized care plans.
- Perform medication reconciliation and coordinate care with medical teams.
- Monitor patient progress and adjust care plans to meet evolving needs.
- Educate patients and caregivers on disease management and symptom control strategies.
- Ensure timely and accurate documentation in compliance with standards.
- Supervise Home Health Aides and conduct required supervisory visits.
- Participate in interdisciplinary team meetings and case conferences.
- Maintain compliance with insurance authorizations and payer requirements.
Job Requirements
- Active New Jersey Registered Nurse (RN) License and AHA BLS Certification.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing accepted.
- Minimum of 1 year RN experience or 6 months with LPN background.
- Proficiency in computer systems and documentation processes.
- Experience in home care, hospice, or community health is advantageous.
- Strong organizational skills and ability to manage multiple patient cases.

- Willingness to work evening shifts and participate in weekend rotations.
