Job Description

Overview

  • Provide compassionate hospice care to patients in home and assisted living settings.
  • Manage a caseload of 15–20 patients weekly, ensuring quality care delivery.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Educate patients and caregivers on disease progression and symptom management.
  • Coordinate care across interdisciplinary teams and healthcare providers.
  • Ensure compliance with documentation standards and regulatory requirements.
  • Participate in case conferences and contribute to care planning discussions.
  • Offer professional development opportunities and a flexible work environment.

Key Responsibilities & Duties

  • Deliver skilled nursing care to hospice patients, including symptom management and personal care.
  • Perform medication reconciliation and collaborate with physicians and team members.
  • Monitor patient progress and adjust care plans to meet evolving needs.
  • Educate patients and families on medications, disease management, and end-of-life care.
  • Document care activities accurately and timely in compliance with standards.
  • Supervise Home Health Aides and conduct required supervisory visits.
  • Coordinate services and act as a liaison between patients and healthcare providers.
  • Participate in interdisciplinary team meetings and case conferences.

Job Requirements

  • Active New Jersey Registered Nurse (RN) License and AHA BLS Certification.
  • Bachelor of Science in Nursing (BSN) preferred; Associate Degree or Diploma required.
  • Minimum of 6 months RN or 1 year LPN experience.
  • Proficiency in computer systems and documentation processes.
  • Experience in Home Care, Hospice, or Community Health is advantageous.
  • Strong communication and coordination skills for interdisciplinary collaboration.
