Primary Care Physician

Key Responsibilities & Duties

• Diagnose, treat, and manage acute and chronic conditions with a focus on adult and geriatric care.
• Develop and implement individualized care plans in collaboration with patients and healthcare teams.
• Provide health education and coaching to promote sustainable lifestyle changes and wellness.
• Lead and supervise clinical teams to ensure high-quality care delivery and patient satisfaction.
• Engage with transitional care teams to facilitate seamless patient care transitions.
• Participate in quality improvement initiatives and adhere to healthcare compliance standards.
• Contribute to the growth of the healthcare center through patient and provider engagement.
• Apply evidence-based practices to optimize patient health and overall wellness.

Job Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics is mandatory.
• Active state medical license or eligibility to obtain one promptly is necessary.
• Minimum of 1 year of clinical experience required; 5 years preferred for senior-level expertise.
• Proficiency in healthcare documentation systems and Microsoft Office Suite is essential.
• Basic Life Support (BLS) certification required within 90 days of employment.
• Strong commitment to delivering patient-centered care in a mission-driven environment.
• Willingness to travel locally and nationally for training and recruitment purposes.
