Key Responsibilities & Duties

  • Perform comprehensive well-woman exams, addressing preventive health needs and providing education on contraception options.
  • Insert and manage long-acting reversible contraception devices, ensuring patient comfort and satisfaction.
  • Assess patient health needs and develop actionable plans for effective chronic disease management.
  • Coordinate care transitions for patients with complex medical and mental health conditions, ensuring seamless support.
  • Promote preventive care strategies through patient education and evidence-based practices to enhance health outcomes.

Job Requirements

  • Master of Science in Nursing degree with active licensure as a Family Nurse Practitioner (FNP).
  • Minimum of 1 year of experience in women's health or primary care; 2 years preferred.
  • Proficiency in conducting well-woman exams and managing preventive health needs effectively.
  • Experience in inserting and managing long-acting reversible contraception devices.
  • Strong knowledge of chronic disease management and evidence-based healthcare practices.
  • Ability to collaborate effectively within interdisciplinary teams and engage with caregivers to optimize care.
