Key Responsibilities & Duties

• Deliver nursing coverage across multiple sites, ensuring seamless operations and patient care continuity.
• Assess site-specific needs and prioritize tasks to address immediate healthcare requirements.
• Collaborate with site leadership and interdisciplinary teams to enhance care delivery.
• Provide patient-centered care while adapting to diverse workflows and populations.
• Maintain accurate documentation of patient interactions and care activities.
• Participate in organizational projects and initiatives during non-coverage periods.
• Travel between designated sites in Brooklyn, Manhattan, and Queens as required.
• Proactively seek guidance and clarification to ensure effective performance.

Job Requirements

• Active Licensed Practical Nurse (LPN) license in good standing.
• Associate of Applied Science (AAS) degree in nursing required.
• Minimum of 1 year of nursing experience, with 3 years preferred.
• Strong organizational and time management skills to handle dynamic environments.
• Ability to adapt quickly to new settings and work independently.
• Excellent communication and interpersonal skills for effective team collaboration.
• Comfortable traveling between sites in Brooklyn, Manhattan, and Queens.
• Experience in community health, outpatient, or behavioral health settings preferred.
