Hospice Team Manager

Overview

• Lead a dedicated team of hospice professionals in a hybrid work setting.
• Ensure exceptional delivery of hospice and palliative care services to patients and families.
• Collaborate with interdisciplinary teams to enhance patient outcomes and support systems.
• Provide expert guidance on pain management and symptom control strategies.
• Oversee compliance with healthcare regulations and documentation standards.
• Participate in community outreach to promote hospice services and awareness.
• Support onboarding and continuous training for new and existing staff members.
• Adapt to flexible scheduling, including rotating weekends as required.

Key Responsibilities & Duties

• Supervise and mentor a team of nursing professionals to achieve excellence in care delivery.
• Coordinate staff assignments ensuring optimal patient-to-staff ratios and adherence to policies.
• Monitor and ensure quality and compliance of nursing documentation with regulations.
• Empower RN Case Managers to excel in patient care and symptom management.
• Participate in interdisciplinary team meetings to contribute to comprehensive care planning.
• Facilitate community presentations to enhance hospice service awareness.
• Support orientation and quarterly training sessions for staff development.
• Ensure adherence to organizational policies and procedures in daily operations.

Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited institution.
• Minimum of five years of hospice nursing experience required.
• Supervisory or leadership experience in a healthcare setting preferred.
• Active New York State RN licensure required.
• National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
• Proficiency in Word, Outlook, and Windows applications.
• Strong ability to supervise and mentor nursing staff effectively.
• Capability to work collaboratively within interdisciplinary teams.
