Overview

• Provide compassionate medical care and case management for clients in supportive housing, addressing diverse medical and psychiatric needs.
• Collaborate with interdisciplinary teams to create tailored wellness plans and promote client independence.
• Conduct health assessments, monitor vital signs, and ensure accurate documentation for all client interactions.
• Coordinate healthcare services, psychiatric support, and community resources to meet individual client needs.
• Provide health education and guidance to empower clients in managing their conditions effectively.
• Respond to emergencies with prompt first aid and CPR, ensuring client safety and well-being.

Key Responsibilities & Duties

• Serve as the primary medical case manager, ensuring comprehensive care coordination for assigned clients.
• Conduct regular health assessments and wellness checks to monitor and address client conditions.
• Collaborate with healthcare providers, pharmacies, and psychiatric professionals to meet client needs.
• Support clients in accessing community resources and navigating daily living challenges.
• Maintain detailed documentation of client interactions, medication administration, and treatment plans.
• Participate in interdisciplinary team meetings to align on client care strategies and progress.
• Provide health education to clients, advocating for their healthcare goals and independence.
• Respond promptly to emergencies, delivering first aid and CPR as required.

Job Requirements

• Active Licensed Practical Nurse (LPN) license is required for this role.
• Associate of Applied Science (AAS) degree in nursing or a related field is mandatory.
• Minimum of 1 year of relevant experience; 3 years preferred for optimal proficiency.
• Strong interest in behavioral health, supportive housing, and community-based care initiatives.
• Proficiency in communication, documentation, and organizational skills is essential.
• Ability to work effectively with vulnerable populations facing medical and psychiatric challenges.
