Overview

  • Provide compassionate medical care and case management for clients in supportive housing, addressing diverse medical and psychiatric needs.

  • Conduct health assessments, monitor vital signs, and ensure accurate documentation for all client interactions.

  • Respond to emergencies with prompt first aid and CPR, ensuring client safety and well-being.

Key Responsibilities & Duties

  • Serve as the primary medical case manager, ensuring comprehensive care coordination for assigned clients.

Job Requirements
