Key Responsibilities & Duties

  • Administer prescribed medications and treatments, ensuring accuracy and patient safety.
  • Monitor patient conditions, promptly reporting changes to the healthcare team.
  • Document patient care activities and maintain detailed medical records.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Provide emotional support and health education to patients and their families.
  • Ensure adherence to infection control and safety protocols in all care activities.

Job Requirements

  • Valid Licensed Practical Nurse (LPN) license in the state of practice.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) and IV certification required.
  • Experience working with geriatric populations is preferred but not mandatory.
  • High School Diploma or GED is required.
  • Commitment to working rotating weekend shifts as part of the schedule.
  • Strong communication and interpersonal skills to effectively interact with patients and colleagues.
