Job Description

Overview

  • Coordinate and complete referrals ordered by health center providers, ensuring seamless patient care and communication.
  • Verify third-party information for referrals and arrange referral appointments with external specialists.
  • Track referral outcomes and collect consult reports from specialists for accurate record-keeping.
  • Collaborate with Patient Care Coordinators to ensure referral processes are efficiently managed.
  • Maintain structured data fields in electronic medical records for referral tracking.
  • Provide coverage at alternate locations and work evenings or weekends as needed.
  • Participate in daily morning huddles to update referral statuses for clients.
  • Adhere to organizational core values and contribute to advocacy events.

Key Responsibilities & Duties

  • Coordinate all aspects of patient referrals, ensuring alignment with primary care providers.
  • Maintain communication with providers and patients to identify appropriate specialists.
  • Ensure patients receive necessary paperwork for appointments, including lab orders and progress notes.
  • Track referral outcomes in medical records and notify ordering providers of results.
  • Secure insurance authorizations for referrals and appointments as required.
  • Maintain a comprehensive list of specialty providers and accepted payor sources.
  • Participate in team meetings to discuss referral statuses and client updates.
  • Provide coverage at alternate locations and adapt to flexible scheduling needs.

Job Requirements

  • High School Diploma or GED required; experience in medical-related fields preferred.
  • Proficiency in Microsoft Office Suite, Outlook, and internet applications.
  • Strong organizational skills and attention to detail for managing multiple priorities.
  • Ability to work with culturally diverse populations and maintain interpersonal relationships.
  • Willingness to work occasional evenings and weekends with advance notice.
  • High level of interpersonal skills for handling sensitive and confidential client information.
