- Collaborate with Patient Care Coordinators to ensure referral processes are efficiently managed.
- Maintain structured data fields in electronic medical records for referral tracking.
- Provide coverage at alternate locations and work evenings or weekends as needed.
- Participate in daily morning huddles to update referral statuses for clients.

Key Responsibilities & Duties

Job Requirements
