Overview
- Deliver high-quality nursing care to patients in their homes, ensuring personalized and effective treatment plans.
- Perform comprehensive physical assessments to address diverse patient needs and conditions.
- Collaborate with multidisciplinary teams to develop and adjust patient care plans.
- Provide education and counseling to patients and families for better health management.

- Document all care activities accurately and promptly for effective communication and compliance.

- Travel within the designated area to provide on-site nursing care and assessments.
Key Responsibilities & Duties
- Conduct thorough physical assessments to evaluate patient health and identify care needs.
- Develop, implement, and revise individualized care plans to achieve optimal outcomes.
- Administer skilled nursing procedures, including preventive and rehabilitative care.
- Monitor and report changes in patient conditions to healthcare providers promptly.
- Educate patients and families on health management practices and care protocols.
- Participate actively in interdisciplinary care planning meetings.
- Lead orientation sessions for aides to ensure consistent and high-quality care delivery.
- Travel within the field to provide direct supervision and nursing care to patients.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Active Registered Nurse (RN) license with at least 1-2 years of field nursing experience.
- Proficiency in conducting physical assessments and developing care plans.
- Experience with patients with Dementia or Tuberculosis is preferred.
- Strong documentation skills for accurate and timely record-keeping.
- Ability to educate and counsel patients and families effectively.
- Preferred experience with Licensed Clinical Social Worker Assistant (LCSA).
- Flexibility to work part-time, 2-3 days per week, with a dynamic schedule.
