Key Responsibilities & Duties

  • Conduct thorough health assessments and monitor vital signs to ensure client safety and well-being.
  • Administer medications accurately and maintain detailed records in compliance with established protocols.
  • Provide emergency first aid and CPR assistance when necessary to address urgent needs.
  • Educate clients on health management strategies and advocate for their preferences and needs.
  • Collaborate with team members during case conferences and treatment planning sessions to ensure comprehensive care.
  • Assist clients with daily living skills and support their recovery goals through personalized care plans.

Job Requirements

  • Active LPN license with a minimum of 1 year of relevant nursing experience.
  • Preferred 3 years of experience in medical or psychiatric care settings for enhanced expertise.
  • Associate of Science (AS) degree in nursing or a related healthcare field.
  • Proficiency in health education, advocacy, and support for diverse populations.
  • Experience with accurate documentation and medication record maintenance in compliance with protocols.
  • Strong problem-solving skills and the ability to manage complex cases effectively.
  • Capability to work collaboratively within a multidisciplinary team environment to achieve shared goals.
