Primary Care/Women's Health Family Nurse Practitioner Overview

  • Deliver exceptional healthcare as a Family Nurse Practitioner specializing in Women's Health and Primary Care.
  • Provide patient-centered care addressing diverse medical needs across all life stages.
  • Conduct preventive screenings, chronic disease management, and well-woman exams with a holistic approach.
  • Collaborate with multidisciplinary teams to ensure optimal patient outcomes and care coordination.
  • Engage in clinic-based and in-home patient care to enhance healthcare accessibility.
  • Utilize evidence-based practices to address primary care and gynecological concerns effectively.
  • Support patients through various health transitions, including pregnancy, menopause, and chronic condition management.
  • Ensure accurate and timely documentation within the Electronic Medical Record (EMR) system.

Primary Care/Women's Health Family Nurse Practitioner Key Responsibilities & Duties

  • Perform comprehensive patient assessments, including history and physical exams, to evaluate health risks and conditions.
  • Provide contraceptive counseling and perform advanced procedures such as IUD insertions.
  • Diagnose and treat sexually transmitted infections and deliver sexual health education.
  • Develop personalized care plans for chronic conditions like diabetes and hypertension.
  • Coordinate specialized care with prenatal and other medical specialists as needed.
  • Screen for social determinants of health and connect patients with supportive resources.

Primary Care/Women's Health Family Nurse Practitioner Job Requirements

  • Master of Science in Nursing with Family Nurse Practitioner certification.
  • Active RN licensure and DEA registration in the state of practice.
  • Minimum of 1 year of experience in Women's Health and Primary Care; 2 years preferred.
  • Proficiency in phlebotomy, immunizations, and office-based medical procedures.
  • Experience addressing healthcare barriers for underserved populations.
  • Fluency in an additional language is highly desirable.
