Evening Hospice Registered Nurse Overview
- Provide compassionate care as an Evening Hospice Registered Nurse, supporting patients during critical evening hours.
- Deliver personalized end-of-life care, focusing on symptom management and emotional support.
- Collaborate with interdisciplinary teams to create and implement tailored care plans.
- Educate patients and families on disease progression and symptom control strategies.
- Perform clinical tasks including wound care, catheter management, and vital sign monitoring.

Evening Hospice Registered Nurse Key Responsibilities & Duties
- Deliver skilled nursing care to hospice patients in home and community settings.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Perform medication reconciliation and coordinate care with medical teams.
- Monitor patient progress and adjust care plans to meet evolving needs.
- Educate patients and caregivers on disease management and symptom control strategies.

Evening Hospice Registered Nurse Job Requirements
- Active New Jersey Registered Nurse (RN) License and AHA BLS Certification.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing accepted.
- Minimum of 1 year RN experience or 6 months with LPN background.
- Proficiency in computer systems and documentation processes.
- Experience in home care, hospice, or community health is advantageous.
- Strong organizational skills and ability to manage multiple patient cases.
- Commitment to providing compassionate and patient-centered care.
- Willingness to work evening shifts and participate in weekend rotations.
