Field Hospice Registered Nurse Overview
- As a Field Hospice Registered Nurse, deliver compassionate care to patients in their homes and assisted living facilities.
- Manage a caseload of 15–20 patients weekly, ensuring personalized and high-quality care delivery.
- Conduct comprehensive assessments and develop individualized care plans tailored to patient needs.
- Educate patients and caregivers on disease progression, symptom management, and end-of-life care.
- Collaborate with interdisciplinary teams to coordinate seamless care and enhance patient outcomes.
- Ensure compliance with documentation standards and regulatory requirements to maintain care quality.

Field Hospice Registered Nurse Key Responsibilities & Duties
- Deliver skilled nursing care, including symptom management and personal care, to hospice patients.

- Educate patients and families on medications, disease management, and end-of-life care practices.

Field Hospice Registered Nurse Job Requirements
- Active New Jersey Registered Nurse (RN) License and AHA BLS Certification required.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree or Diploma required.
- Minimum of 6 months RN or 1 year LPN experience in a clinical setting.
- Proficiency in computer systems and documentation processes is essential.
- Experience in Home Care, Hospice, or Community Health is highly advantageous.
- Strong communication and coordination skills for effective interdisciplinary collaboration.
