Hospice Social Worker Key Responsibilities & Duties

• Provide psychosocial support to hospice patients and their families in various care settings.
• Develop and implement social work care plans based on patient and family needs.
• Collaborate with interdisciplinary teams to ensure holistic patient care.
• Serve as a liaison between hospice staff and inpatient/nursing home staff.
• Support patients in pre- and post-bereavement phases with counseling and resources.
• Ensure compliance with all regulatory and organizational policies.
• Maintain accurate and timely clinical documentation.
• Participate in training and supervision to enhance professional skills.
• Provide weekend coverage as needed to support patient care continuity.

Hospice Social Worker Job Requirements

• Master of Social Work degree from an accredited institution.
• New York State licensure as LMSW or LCSW is mandatory.
• Minimum one year of social work experience in a healthcare setting.
• Experience with hospice or palliative care is highly preferred.
• Proficiency in Microsoft Word and Excel is advantageous.
