Physical Therapist Key Responsibilities & Duties

• Conduct in-home evaluations and develop customized treatment plans tailored to individual patient needs.
• Administer therapy sessions focusing on mobility, balance, strength, and pain management.
• Collaborate with referring physicians and healthcare teams to ensure integrated patient care.
• Educate patients and caregivers on effective home management strategies to enhance recovery.
• Maintain accurate and timely documentation across multiple EMR systems.
• Coordinate schedules with patients, ensuring flexibility and adherence to therapy goals.
• Perform evaluations, reassessments, and discharge planning to optimize patient outcomes.
• Support routine visits through collaboration with Physical Therapist Assistants.

Physical Therapist Job Requirements

• Active Florida Physical Therapist license and valid driver’s license required.
• Minimum of 1 year of experience in home health therapy preferred.
• Proficiency in utilizing multiple EMR systems for patient documentation.
• Strong organizational skills and ability to manage flexible schedules effectively.
• Professional demeanor and commitment to delivering exceptional patient care.
• Reliable transportation to travel within assigned coverage areas.
• High School Diploma or GED required; advanced qualifications are a plus.
