- The Registered Nurse Case Manager Hospice will provide compassionate care to patients in end-of-life stages, ensuring their comfort and dignity.
- Develop and implement individualized nursing care plans based on patient assessments and needs.
- Collaborate with interdisciplinary teams to deliver holistic care and support.
- Supervise and train home health aides, homemakers, and personal care workers in patient homes.
- Request and manage physician orders for care plan adjustments as necessary.
- Provide education and guidance to patients and families regarding hospice care and processes.

Registered Nurse Case Manager Hospice Key Responsibilities & Duties
- Conduct comprehensive physical and psychosocial assessments for hospice patients.

Registered Nurse Case Manager Hospice Job Requirements
- Associate of Science (AS) degree in Nursing or equivalent education.
- Minimum of 2 years of clinical experience, preferably in hospice, home care, or acute care settings.
- Strong ability to work within interdisciplinary team environments.
- Proficiency in clinical data input and documentation.
- Excellent communication, teaching, and listening skills.
- Ability to supervise and evaluate home care staff effectively.
- Commitment to providing compassionate and patient-centered care.
- Knowledge of hospice care standards and regulations is preferred.
- Willingness to work on-site and travel to patient homes as needed.
