Coordinator of Care Key Responsibilities & Duties

• Conduct initial patient evaluations to create tailored home health care plans addressing individual needs and preferences.
• Coordinate care services to minimize risks of rehospitalization and enhance patient recovery outcomes.
• Provide bedside nursing care to patients in home settings, ensuring comfort and quality care delivery.
• Collaborate with physicians, caregivers, and family members to ensure seamless care delivery and communication.
• Review insurance coverage and ensure compliance with patient care plans and organizational protocols.
• Participate in interdisciplinary discussions to enhance patient care strategies and outcomes.
• Manage occasional on-call duties to provide timely and effective patient support as needed.
• Complete necessary training and certifications to ensure readiness for all responsibilities and maintain professional standards.

Coordinator of Care Job Requirements

• Associate of Applied Science (AAS) degree required; Bachelor of Science in Nursing (BSN) preferred.
• Registered Nurse (RN) certification mandatory with active licensure in the relevant jurisdiction.
• Minimum of 2 years of experience in hospital, acute care, Med/Surg, or home care settings.
• Field-based role requiring a valid driver’s license and reliable transportation for patient visits.
• Proficiency in case management and interdisciplinary care coordination to ensure optimal outcomes.
• Strong communication skills to interact effectively with patients, families, and healthcare teams.
• Commitment to delivering exceptional care and achieving positive patient outcomes consistently.
