Position Summary
The Remote Case Manager is responsible for coordinating and managing patient care remotely, ensuring that patients receive comprehensive and continuous care. This role involves assessing patient needs, developing care plans, and facilitating access to necessary services. The Remote Case Manager collaborates with healthcare providers, patients, and their families to achieve optimal health outcomes.
Essential Job Duties
- Conduct remote assessments of patient needs and develop individualized care plans.
- Coordinate care and services for patients, including referrals to specialists and community resources.
- Monitor patient progress and adjust care plans as necessary.
- Provide patient education and support, including information on treatment options and self-care strategies.
- Facilitate communication between patients, families, and healthcare providers.
- Document patient interactions and care plans accurately and timely.
- Participate in multidisciplinary team meetings to discuss patient care and progress.
- Utilize telehealth technologies to deliver care and support to patients.
- Ensure compliance with all relevant regulations and standards.
