Overview

  • Join a dynamic team as a Social Worker in Savannah, GA, focusing on community health and patient advocacy.
  • Work within an interdisciplinary care integration team to address barriers to health and increase access to care.
  • Engage patients proactively, serving as a link between them, their community, and healthcare providers.
  • Implement targeted interventions to support patients' health goals and self-management.
  • Conduct outreach activities, including home visits, to identify healthcare needs and barriers.
  • Provide culturally appropriate health education and advocate for patient needs.
  • Coordinate care across health and social service systems, ensuring seamless communication.
  • Empower patients to engage with community resources and build supportive relationships.

Key Responsibilities & Duties

  • Assess community health needs and identify priority areas for intervention.
  • Conduct health screenings and coordinate with healthcare providers for further evaluation.
  • Facilitate communication between patients, healthcare providers, and social service agencies.
  • Advocate for patients to access necessary healthcare services and address barriers.
  • Provide emotional support and coaching to individuals navigating complex health situations.
  • Document patient encounters in medical record systems accurately and timely.
  • Participate in interdisciplinary team rounds and care planning for high-risk members.
  • Assist patients with transportation, food insecurity, and benefits applications.

Job Requirements

  • Associate of Science (AS) degree required; Bachelor’s degree preferred.
  • Minimum of 2 years of experience in human services and community-based resources.
  • Community Health Worker certification preferred but not required.
  • Bilingual in English and Spanish or Creole with proficient communication skills.
  • Experience working with seniors and conducting home visits.
  • Strong organizational, interpersonal, and communication skills.
  • Valid driver’s license, reliable transportation, and insurance coverage.
  • Ability to work on-site and travel locally for patient visits.
