Overview

  • Join a dynamic healthcare team as a Part-Time Family Nurse Practitioner, providing primary care services to youth in supervised independent living and group residences.
  • Deliver trauma-informed care, emphasizing harm reduction and motivational interviewing, to support the physical and mental well-being of clients.
  • Collaborate with multidisciplinary teams, including mental health clinicians, to ensure holistic health outcomes for clients.
  • Work flexibly across two locations, with options for telehealth sessions, ensuring accessibility and convenience.
  • Provide medical assessments, treatment planning, and health education to foster care youth and residents.
  • Stay updated with advancements in primary care practices to deliver innovative and effective healthcare solutions.
  • Engage in practice improvement projects to enhance service quality and client satisfaction.
  • Utilize electronic health records for accurate and prompt documentation of medical services provided.

Key Responsibilities & Duties

  • Conduct comprehensive primary care services, including histories, physical exams, and diagnoses, emphasizing preventative care and screenings.
  • Order and interpret diagnostic tests, formulating treatment plans based on results and clinical guidelines.
  • Prescribe medications and recommend therapeutic interventions to address acute and chronic conditions effectively.
  • Educate clients on health conditions, preventative measures, and therapeutic options to promote self-care and wellness.
  • Collaborate with case managers, social workers, and psychiatric providers to integrate mental and physical health care.
  • Maintain accurate medical records using the organization’s electronic health record system.
  • Participate in quality improvement initiatives to enhance healthcare delivery and client outcomes.
  • Provide health counseling and education, including sexual and reproductive health, tailored to individual client needs.

Job Requirements

  • Possess a valid Family Nurse Practitioner (FNP) certification and relevant licensure.
  • Minimum of one year of experience in a clinical setting, preferably working independently.
  • Proficient in trauma-informed care, harm reduction, and motivational interviewing techniques.
  • Experience with electronic health records and maintaining accurate medical documentation.
  • Strong understanding of the interplay between mental health and physical health conditions.
  • Ability to work flexibly across multiple locations and adapt to telehealth services.
