Job Description

Overview

  • Provide physical therapy services to geriatric patients in a home health setting.
  • Collaborate with multiple home health agencies to deliver quality care.
  • Manage a flexible schedule with patient appointment coordination.
  • Utilize various EMR systems for documentation and scheduling.
  • Perform evaluations, assessments, and discharge planning for patients.
  • Work within a 13-week contract with potential for extension.
  • Engage in virtual orientation and training for seamless onboarding.
  • Contribute to a team serving patients across Brevard County.

Key Responsibilities & Duties

  • Conduct comprehensive physical therapy evaluations and treatment plans.
  • Document patient progress in EMR systems such as Pariox, Kantime, and Axxess.
  • Coordinate schedules with patients and update EMRs accordingly.
  • Collaborate with PTAs for routine visits and focus on specialized assessments.
  • Provide therapy services across different regions of Brevard County.
  • Ensure compliance with healthcare regulations and standards.
  • Participate in virtual training sessions for role preparation.
  • Maintain effective communication with home health agencies and patients.

Job Requirements

  • Bachelor’s degree in Physical Therapy or equivalent educational background.
  • Minimum of 1 year of experience in physical therapy services.
  • Proficiency in EMR systems such as Pariox, Kantime, and Axxess.
  • Ability to manage flexible schedules and coordinate patient appointments.
  • Experience with geriatric patient care in home health settings.
  • Strong communication and documentation skills.
  • Availability for a 13-week contract with potential for extension.
  • Willingness to work on-site across Brevard County regions.
