Job Description
Overview
- Provide physical therapy services to patients in Brevard County, ensuring high-quality care and patient satisfaction.
- Work on a contract basis with a flexible schedule, including potential Saturday coverage.
- Collaborate with home health agencies to deliver therapy services and document patient progress in various EMR systems.
- Manage a steady caseload with approximately 30 visits per week, adapting to patient needs and agency requirements.
- Coordinate daily schedules with patients, ensuring timely communication and efficient appointment setting.
- Utilize Pariox EMR for centralized scheduling and documentation, alongside agency-specific systems.
- Conduct evaluations, SOCs, RAs, and DCs, delegating routine visits to PTAs when possible.
- Participate in virtual orientation and training to ensure seamless integration into the team.
Key Responsibilities & Duties
- Perform physical therapy evaluations, treatments, and discharge planning for geriatric patients in home health settings.
- Document patient progress and therapy outcomes in agency-specific EMR systems, ensuring compliance with standards.
- Coordinate with schedulers to finalize daily schedules and communicate with patients for appointment times.
- Collaborate with home health agencies to provide therapy services during staff shortages or as primary therapy providers.
- Manage caseload efficiently, balancing patient needs and documentation requirements.
- Utilize Pariox EMR for centralized scheduling and documentation, ensuring streamlined operations.
- Provide mentorship and guidance to PTAs, delegating routine visits appropriately.
- Participate in virtual training sessions to stay updated on protocols and procedures.
Job Requirements
- Bachelor’s degree in Physical Therapy or equivalent education and licensure.
- Minimum of 1 year of experience in physical therapy, preferably in home health settings.
- Proficiency in EMR systems such as Kantime, Kinnser, Axxess, and Homecare Homebase.
- Strong organizational skills to manage caseloads and coordinate schedules effectively.
- Ability to work independently and adapt to diverse patient needs and agency protocols.
- Flexibility to cover various locations within Brevard County, including potential Saturday visits.
- Commitment to providing high-quality care and maintaining patient satisfaction.
