Key Responsibilities & Duties

• Perform patient assessments to identify social, psychological, and environmental needs.
• Develop and implement individualized care plans to address identified needs.
• Coordinate with community resources to provide housing, food, and mental health support.
• Maintain accurate documentation of patient interactions and care plans.
• Collaborate with primary care providers and other healthcare professionals to ensure integrated care.
• Conduct follow-up visits remotely to monitor patient progress and adjust care plans.

• Ensure compliance with Medicaid regulations and organizational policies.

Job Requirements

• Master of Social Work degree and Licensed Clinical Social Worker certification.
• Minimum of 1 year of experience, with 3 years preferred in social work or related fields.
• Knowledge of Medicaid regulations and processes is essential.
• Valid and clean driver’s license for field visits and patient interactions.
• Strong ability to conduct assessments and connect patients to appropriate resources.
• Proficiency in maintaining detailed documentation and care plans.
