Overview

  • Provide high-quality medical care through in-home and virtual visits, ensuring comprehensive assessments, acute visits, and follow-ups for diverse patient needs.
  • Collaborate with clinical teams to create individualized treatment plans, embracing a patient-centric model of care and promoting value-based healthcare practices.
  • Utilize advanced telemedicine tools for remote diagnostics and therapeutic procedures, ensuring effective and efficient patient care delivery.
  • Autonomously manage patient caseloads, scheduling appointments flexibly based on patient availability and clinical requirements.
  • Participate in virtual and shadow training sessions to enhance clinical skills and ensure seamless integration into the healthcare team.
  • Support the practice management system and electronic medical record (EMR) for accurate documentation and streamlined operations.
  • Provide on-call backup coverage for clinical escalations during overnight, holiday, and weekend hours across multiple states.
  • Contribute to the organization's mission to improve health and well-being in high-need urban and rural communities.

Key Responsibilities & Duties

  • Conduct comprehensive assessments, acute visits, and follow-ups for patients, utilizing both in-home and virtual care methods.
  • Collaborate with community health workers and clinical teams to ensure effective patient care coordination and scheduling.
  • Perform diagnostic and therapeutic procedures within clinical discipline, consulting supervising physicians as necessary.
  • Develop and implement individualized treatment plans, promoting patient-centric and value-based care models.
  • Utilize telemedicine technologies for remote diagnostics and procedures, ensuring quality care delivery.
  • Maintain proficiency in practice management systems and electronic medical records for accurate documentation.
  • Provide on-call support for clinical escalations during non-standard hours, ensuring continuous patient care.
  • Participate in training programs to enhance clinical skills and knowledge, including virtual and shadow sessions.

Job Requirements

  • Master of Science in Nursing from an accredited institution; active unrestricted medical license in New York.
  • Minimum of 1 year of experience, with 3 years preferred; DEA certification required or willingness to obtain.
  • Proficiency in telemedicine technologies and electronic medical record systems for effective patient care.
  • Strong communication skills for clear articulation of concepts and effective collaboration with teams.
  • Ability to work autonomously and manage patient caseloads effectively, ensuring high-quality care delivery.
  • Experience in home care, hospital, or nursing home settings preferred; comfortable with travel up to an hour.
