Overview

  • Provide compassionate social work support to patients and families facing serious illnesses in a home-based care setting.
  • Collaborate with an interdisciplinary team to deliver holistic, patient-centered care that honors individual values and preferences.
  • Assist patients and caregivers with navigating healthcare systems and accessing community resources.
  • Support emotional, social, and spiritual needs, addressing grief and end-of-life planning.
  • Contribute to care transitions and complex medical decision-making processes.
  • Maintain compliance with documentation standards in electronic medical records.
  • Participate in shared weekend rotation to ensure consistent patient care coverage.

Key Responsibilities & Duties

  • Conduct thorough assessments to identify patient strengths, needs, risks, and care goals.
  • Provide emotional and social support to patients and families coping with serious illnesses.
  • Facilitate discussions on advance directives, goals of care, and end-of-life planning.
  • Assist with referrals to community resources, financial aid, and long-term care services.
  • Collaborate with interdisciplinary teams to develop and implement holistic care plans.
  • Advocate for patient preferences, ensuring culturally sensitive and value-aligned care.
  • Provide crisis intervention during acute changes in patient conditions or caregiving challenges.
  • Ensure accurate and timely documentation in compliance with organizational policies.

Job Requirements

  • Master's degree in Social Work (MSW) from an accredited institution, LMSW required.
  • Preferred experience in hospice, palliative care, or medical social work.
  • Knowledge of serious illness, grief, bereavement, and family dynamics.
  • Strong communication, counseling, and clinical documentation skills.
  • Ability to work independently and collaborate within interdisciplinary teams.
  • Valid driver’s license and reliable transportation for home visits.
  • Schedule flexibility, including participation in weekend rotations.
