Job Description

Overview

  • Provide expert case management services in a healthcare setting, ensuring optimal patient care and resource utilization.
  • Collaborate with multidisciplinary teams to develop and implement patient care plans tailored to individual needs.
  • Work onsite to support emergency department, NICU, and maternal health units.
  • Engage with patients and families to provide guidance and support during their healthcare journey.
  • Ensure compliance with healthcare regulations and standards in all case management activities.
  • Maintain accurate and timely documentation of patient interactions and care plans.
  • Contribute to continuous improvement initiatives within the healthcare organization.

Key Responsibilities & Duties

  • Conduct comprehensive assessments to determine patient needs and develop individualized care plans.
  • Coordinate with healthcare providers to ensure seamless delivery of services across departments.
  • Monitor patient progress and adjust care plans as necessary to achieve desired outcomes.
  • Facilitate communication between patients, families, and healthcare teams to address concerns and provide solutions.
  • Provide coverage in emergency, NICU, and maternal health units during designated shifts.
  • Educate patients and families about available resources and healthcare options.
  • Participate in team meetings and contribute to case discussions and decision-making processes.

Job Requirements

  • Associate of Applied Science (AAS) degree in Nursing or related field required.
  • Minimum of 2 years of experience in case management or related nursing roles.
  • Preferred 3 years of experience in a healthcare setting, particularly in emergency or maternal health units.
  • Strong knowledge of healthcare regulations and patient care standards.
  • Excellent communication and interpersonal skills to interact effectively with patients and teams.
  • Ability to work onsite during scheduled shifts, including weekends and evenings.
  • Proficiency in maintaining accurate and timely documentation.
