Overview
- Provide expert case management services in a healthcare setting, ensuring optimal patient care and resource utilization.
- Collaborate with multidisciplinary teams to develop and implement patient care plans tailored to individual needs.
- Work onsite to support emergency department, NICU, and maternal health units.
- Engage with patients and families to provide guidance and support during their healthcare journey.
- Ensure compliance with healthcare regulations and standards in all case management activities.
- Maintain accurate and timely documentation of patient interactions and care plans.
- Contribute to continuous improvement initiatives within the healthcare organization.
Key Responsibilities & Duties
- Conduct comprehensive assessments to determine patient needs and develop individualized care plans.
- Coordinate with healthcare providers to ensure seamless delivery of services across departments.
- Monitor patient progress and adjust care plans as necessary to achieve desired outcomes.
- Facilitate communication between patients, families, and healthcare teams to address concerns and provide solutions.
- Provide coverage in emergency, NICU, and maternal health units during designated shifts.
- Educate patients and families about available resources and healthcare options.

Job Requirements
- Associate of Applied Science (AAS) degree in Nursing or related field required.
- Minimum of 2 years of experience in case management or related nursing roles.
- Preferred 3 years of experience in a healthcare setting, particularly in emergency or maternal health units.
- Strong knowledge of healthcare regulations and patient care standards.
- Excellent communication and interpersonal skills to interact effectively with patients and teams.
- Ability to work onsite during scheduled shifts, including weekends and evenings.
- Proficiency in maintaining accurate and timely documentation.
