Msw Case Manager

Overview

• Provide case management services to patients, ensuring comprehensive care plans are developed and implemented effectively.
• Collaborate with healthcare professionals to address patient needs and coordinate resources.
• Support patients and families in understanding and navigating healthcare systems and services.
• Conduct assessments to identify social, emotional, and financial needs of patients.
• Develop individualized care plans based on patient needs and goals.
• Ensure compliance with healthcare regulations and standards in all case management activities.
• Maintain accurate and up-to-date documentation of patient interactions and care plans.
• Participate in interdisciplinary team meetings to discuss patient progress and care strategies.

Key Responsibilities & Duties

• Assess patient needs and develop tailored care plans to address identified challenges.
• Coordinate with medical staff and external resources to ensure comprehensive care delivery.
• Provide emotional support and counseling to patients and their families.
• Advocate for patient rights and access to necessary services and resources.

Job Requirements

• Master of Social Work degree from an accredited institution.
• Minimum of 2 years of experience in case management or social work.
• Preferred 3 years of experience in healthcare or related field.
• Strong knowledge of healthcare systems and patient advocacy practices.
• Excellent communication and interpersonal skills for effective collaboration.
• Ability to work on-site and adapt to a dynamic healthcare environment.
• Proficiency in documentation and compliance with healthcare regulations.
• LCSW certification preferred for enhanced compensation opportunities.
