Job Description

Overview

  • Provide comprehensive primary care services to patients, focusing on holistic and value-based care approaches.
  • Work onsite Monday through Friday in a supportive and collaborative healthcare environment.
  • Engage with underserved communities, offering personalized care and addressing unique health challenges.
  • Collaborate with multidisciplinary teams to optimize patient outcomes and health literacy.
  • Utilize advanced technology for efficient documentation and patient management.
  • Participate in professional development and leadership opportunities within the organization.

  • Adhere to regulatory compliance, quality standards, and organizational policies.

Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic illnesses in older adult patients.
  • Lead care teams including medical assistants, care facilitators, and coordinators.
  • Engage with hospitalists for inpatient care coordination and transitional care.
  • Provide health coaching to patients, setting and monitoring health goals.
  • Ensure timely and accurate documentation using electronic health records.
  • Participate in risk and quality management programs and clinical meetings.
  • Recruit patients and providers to enhance center operations and outreach.
  • Supervise and collaborate with Advanced Practice Practitioners (APPs).
  • Promote a culture of accountability, passion, and service orientation within the center.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) in Internal Medicine, Family Medicine, or Geriatrics.
  • Active State Medical License or ability to obtain one within the required timeframe.
  • Board certification in relevant specialty and maintenance of certification.
  • Current DEA number for schedule II-V controlled substances.
  • Basic Life Support (BLS) certification within the first 90 days of employment.
  • Proficiency in Microsoft Office Suite and electronic health record systems.
  • Strong commitment to patient-centered care and community health improvement.
  • Ability to travel locally and nationally for training and recruitment purposes.
  • Fluency in English, with additional language skills preferred for specific centers.
