Overview
- Provide comprehensive primary care services to patients, focusing on holistic and value-based care approaches.
- Work onsite Monday through Friday in a supportive and collaborative healthcare environment.

Key Responsibilities & Duties
- Diagnose, treat, and manage acute and chronic illnesses in older adult patients.
- Lead care teams including medical assistants, care facilitators, and coordinators.
- Engage with hospitalists for inpatient care coordination and transitional care.
- Provide health coaching to patients, setting and monitoring health goals.
- Ensure timely and accurate documentation using electronic health records.
- Participate in risk and quality management programs and clinical meetings.
- Recruit patients and providers to enhance center operations and outreach.
- Supervise and collaborate with Advanced Practice Practitioners (APPs).
- Promote a culture of accountability, passion, and service orientation within the center.
Job Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) in Internal Medicine, Family Medicine, or Geriatrics.
- Active State Medical License or ability to obtain one within the required timeframe.
- Board certification in relevant specialty and maintenance of certification.
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification within the first 90 days of employment.
- Proficiency in Microsoft Office Suite and electronic health record systems.
- Strong commitment to patient-centered care and community health improvement.
- Ability to travel locally and nationally for training and recruitment purposes.
- Fluency in English, with additional language skills preferred for specific centers.
