Overview

Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic conditions in older adult patients.
  • Develop personalized care plans, setting short and long-term health goals with patients.
  • Lead care teams, including medical assistants and care coordinators, to optimize patient outcomes.
  • Ensure compliance with clinical guidelines and participate in quality improvement initiatives.
  • Engage in community outreach and patient recruitment activities to expand service impact.
  • Utilize evidence-based practices and integrate lifestyle interventions for improved health outcomes.
  • Document patient interactions and care plans efficiently using electronic health records.
  • Collaborate with specialists and hospitalists for comprehensive patient care coordination.

Job Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics required.
  • Active state medical license or eligibility to obtain one within the required timeframe.
  • Minimum of 1 year of experience; 5 years preferred in primary care settings.
  • Proficiency in electronic health record systems and Microsoft Office Suite.
  • Strong commitment to patient-centered care and service orientation.
  • Basic Life Support (BLS) certification required within 90 days of employment.
  • Ability to work onsite Monday through Friday, adhering to structured schedules.
