Key Responsibilities & Duties
- Supervise and mentor interdisciplinary teams, ensuring adherence to hospice principles and regulatory standards.
- Manage hiring, scheduling, payroll, and performance evaluations for team members.
- Conduct rounds at healthcare facilities, providing direct support and oversight to staff.
- Lead weekly interdisciplinary team meetings to review patient care plans and eligibility criteria.
- Develop and implement quality improvement projects to enhance team performance and patient satisfaction.
- Collaborate with business development teams to identify and pursue growth opportunities.
- Ensure compliance with CMS, state, and accrediting body regulations for hospice care provision.
Job Requirements
- Bachelor of Science in Nursing (BSN) required; Master’s degree preferred.
- Minimum of two years of hospice nursing experience; supervisory experience preferred.
- CHPN certification required or to be obtained within one year of hire.
- Strong clinical knowledge of hospice regulations and symptom management.
- New York State RN licensure and CPR-BLS certification required.
- Valid NYS driver’s license preferred for travel between locations.
- Proven ability to lead teams, manage priorities, and work flexible hours.
