Job Description

Overview

  • Lead and manage interdisciplinary hospice teams across multiple healthcare facilities, ensuring high-quality patient care and compliance with regulations.
  • Oversee clinical coordinators and liaisons, providing guidance and support to optimize team performance and patient outcomes.
  • Balance clinical and business development responsibilities, fostering relationships and enhancing referral processes.
  • Collaborate with hospital staff and organizational leadership to streamline hospice operations and care delivery.
  • Participate in strategic planning and quality improvement initiatives to elevate hospice services.

Key Responsibilities & Duties

  • Supervise and mentor interdisciplinary teams, ensuring adherence to hospice principles and regulatory standards.
  • Manage hiring, scheduling, payroll, and performance evaluations for team members.
  • Conduct rounds at healthcare facilities, providing direct support and oversight to staff.
  • Lead weekly interdisciplinary team meetings to review patient care plans and eligibility criteria.
  • Develop and implement quality improvement projects to enhance team performance and patient satisfaction.
  • Collaborate with business development teams to identify and pursue growth opportunities.
  • Ensure compliance with CMS, state, and accrediting body regulations for hospice care provision.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; Master’s degree preferred.
  • Minimum of two years of hospice nursing experience; supervisory experience preferred.
  • CHPN certification required or to be obtained within one year of hire.
  • Strong clinical knowledge of hospice regulations and symptom management.
  • New York State RN licensure and CPR-BLS certification required.
  • Valid NYS driver’s license preferred for travel between locations.
  • Proven ability to lead teams, manage priorities, and work flexible hours.
