Job Description

Overview

  • Provide comprehensive assessments for members using the Uniform Assessment System (UAS-NY) to evaluate clinical, psychological, and social needs.
  • Collaborate with interdisciplinary teams to develop individualized care plans based on assessment findings and member preferences.
  • Conduct hybrid assessments, including telephonic and in-home visits, ensuring compliance with regulatory guidelines.
  • Explain program benefits and rules to members and their families in an understandable manner.
  • Ensure accurate documentation and timely submission of assessment outcomes to relevant teams.
  • Complete initial enrollment assessments and reassessments, focusing on safety, eligibility, and care planning.
  • Support members in accessing services, including CDPAS enrollment, and liaise with healthcare professionals.
  • Participate in training sessions to enhance knowledge of policies and procedures.

Key Responsibilities & Duties

  • Conduct UAS-NY assessments to determine member needs and eligibility for services.
  • Develop and communicate individualized care plans to members, families, and care teams.
  • Ensure all assessments are completed within mandated timeframes and documented accurately.
  • Coordinate with interdisciplinary teams to address urgent care needs and safety concerns.
  • Explain program benefits and responsibilities to members during assessments.
  • Conduct follow-up assessments to ensure members receive appropriate care and services.
  • Maintain compliance with DOH policies and regulations during all assessment activities.
  • Participate in training and professional development activities to improve assessment skills.

Job Requirements

  • Associate Degree in Nursing (AAS) required; Bachelor's Degree preferred.
  • Minimum of one year of experience in community health, managed care, or related fields.
  • Proficiency in Microsoft Word, Excel, and UAS-NY systems.
  • Bilingual skills in Bangla, Hindi, Urdu, Mandarin, Cantonese, or Spanish preferred.
  • Valid driver's license and willingness to travel within assigned territories.
  • Comfortable conducting both telephonic and in-home assessments.
  • Strong communication and interpersonal skills to interact with diverse populations.
  • Ability to manage multiple cases and meet productivity targets effectively.
