Key Responsibilities & Duties

  • Conduct detailed in-home assessments to evaluate member health needs and develop personalized care plans.
  • Coordinate care across healthcare providers, ensuring seamless service delivery and quality outcomes.
  • Advocate for member needs, promoting independence and effective utilization of healthcare resources.
  • Authorize covered plan services and monitor member status to ensure consistent care delivery.
  • Collaborate with interdisciplinary teams, including primary care practitioners, to address member health concerns.
  • Communicate effectively with members, families, and caregivers to foster understanding and engagement in care plans.
  • Participate in organizational training and meetings to align with operational protocols and objectives.

Job Requirements

  • Active New York State Registered Nurse license and registration required for practice.
  • Associate of Science in Nursing required; Bachelor of Science in Nursing preferred.
  • Minimum of 3 years of Registered Nurse experience; expertise in geriatrics preferred.
  • Experience in managed long-term care settings highly desirable for effective role performance.
  • Bilingual proficiency in Spanish, Chinese, or Russian required to meet member needs.
  • Strong communication and collaboration skills to work effectively with interdisciplinary teams.
  • Ability to manage a hybrid schedule, balancing remote work and field visits efficiently.
