Overview

- Balance fieldwork and office responsibilities, with 80% field-based case management and 20% office-based professional development tasks.
- Provide direct patient care, carrying a reduced caseload compared to field staff, while supporting team members in their roles.
- Collaborate with the Hospice Nurse Manager to guide staff growth and development through mentorship and supervision.
- Ensure compliance with hospice and palliative care program philosophy, goals, and policies.
- Work in Upper Manhattan territory with rotating weekend shifts; hours typically 8 AM to 5 PM.
- Participate in a two-step interview process, conducted virtually or in person.

Job Requirements
- Graduate from an accredited School of Nursing; BSN preferred.
- Minimum of five years of hospice nursing experience preferred; supervisory or leadership experience is advantageous.
- New York State RN licensure is required; CHPN certification preferred or obtained within one year of hire.
- Proficiency in Word, Outlook, and Windows software for administrative tasks.
- Ability to effectively supervise and mentor hospice nursing staff, fostering their professional growth.
- Strong organizational and communication skills to manage field and office responsibilities.
