Key Responsibilities & Duties
- Conduct home health visits, including patient admissions, re-certifications, and discharges, ensuring high-quality care.
- Develop and coordinate Physician Plans of Care in collaboration with healthcare professionals.
- Document patient care activities accurately and promptly using provided technology tools.
- Perform OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
- Ensure compliance with Department of Health regulations and organizational standards.
- Deliver culturally competent care that aligns with the organization’s mission and values.
- Collaborate with the Clinical Manager and interdisciplinary team to achieve patient recovery goals.
- Provide patient and family education on health management and preventive care strategies.
Job Requirements
- Valid Nursing license and proven experience as a registered nurse.
- Associate of Applied Science (AAS) degree in Nursing or equivalent qualification.
- Experience in acute care, hospital settings, or home care is essential.
- Proficiency in completing comprehensive OASIS assessments is required.
- Pediatric experience is preferred but not mandatory for this role.
- Must possess a reliable vehicle and valid driver’s license for travel.
- Experience with Homecare Homebase (HCHB) software is an advantage.
- Strong communication and organizational skills to manage patient care effectively.
