Job Description

Overview

  • Deliver compassionate nursing care to individuals with developmental disabilities, ensuring their health and well-being in a supportive environment.
  • Coordinate healthcare services, including follow-ups and communication with medical providers to ensure continuity of care.
  • Develop and implement personalized nursing plans tailored to each individual's unique needs and conditions.
  • Monitor health status changes, documenting observations and initiating timely interventions as required.
  • Collaborate with interdisciplinary teams, families, and clinicians to optimize care and support for individuals.
  • Oversee medication administration processes and provide necessary training to staff members.
  • Participate in team meetings to address health concerns and develop improved care strategies.
  • Advocate for individuals during medical appointments, ensuring their healthcare needs are met effectively.
  • Ensure compliance with healthcare regulations and organizational policies to maintain high standards of care.

Key Responsibilities & Duties

  • Conduct thorough health assessments and monitor individuals for signs of illness or discomfort.
  • Document health findings and follow up on treatments or changes in health status.
  • Coordinate medical appointments and advocate for individuals' healthcare needs during consultations.
  • Develop and update nursing care plans based on evolving health conditions of individuals.
  • Train and supervise staff in medication administration and healthcare protocols.
  • Respond promptly to urgent health situations, providing necessary interventions and support.
  • Participate in interdisciplinary meetings to discuss and address health-related issues.
  • Ensure adherence to healthcare regulations and organizational policies in all nursing practices.
  • Support individuals during medical procedures, ensuring their comfort and understanding throughout.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and active RN licensure in New York State.
  • Minimum of 2 years of nursing experience, preferably in developmental disabilities care.
  • CPR certification and ability to manage multiple priorities effectively.
  • Strong communication skills for collaboration with teams, families, and medical providers.
  • Valid NY State or Tri-state area driver's license preferred for travel requirements.
  • Knowledge of healthcare regulations and medication administration protocols.
  • Organizational skills and teamwork capability to ensure effective care delivery.
  • Commitment to advocating for individuals' health and providing high-quality nursing care.
